What happened to the refresh icon for IE toolbar? I had it before Yahoo, but it is now gone.

You didn't specify which version of IE you are using but for IE6 right click on an empty place on the Toolbar, Unlock the toolbar then choose Customize. There you can add or remove the toolbar icons. After making any changes, relock the toolbar, close all browser windows and then reopen again.
